Sunbury Neighbourhood House produces several publications, and film clips including (but not limited to) print publications: Annual Report, promotion materials, and online social media posts including a website, Instagram and Facebook page. SNH often require photographs and film of clients, groups or community members attending programs or participating in events to be used in our online and print publications.
If you consent for SNH to use photographs and film footage — taken by our staff or a photographer we contract—of you, or a person in your care, please check “YES” where given the option on our form. We will also only ever use “First names” in our publications.
If you are under the age of 18 or unable to consent, then your parent guardian or carer must consent on your behalf.
By checking “YES”, you are giving Sunbury Neighbourhood House permission to use photographs and film of you, or a person in your care, in publications (such as those detailed above) without prior approval, acknowledgement or the entitlement to any remuneration now or in the future.
